- You’re giving us permission to write your story and put it on our website.
- You’ll see the finished story first — nothing is published until you say yes.
- You can change your mind later and ask us to take it down.
You’ve been telling your story to Created For Work. This form asks your permission to turn what you shared into a story we can put on our website, created4.work, for other people to read.

What we’ll do with your story
- We’ll take what you shared and write it into a short story.
- We may edit it to make it clear and easy to read. We won’t change what’s true about you.
- You’ll see it first. Before your story goes public, we’ll show you the finished version. You can ask us to change it, or decide you don’t want it shared. Nothing goes on the website until you say yes to the final version.
- Once it’s on our website, other people can read it, and we may share it in our own places (like a newsletter or our own pages) to help and encourage others.
How you’ll be named
- We’ll show your story the way we talked about, including how you want to be named.
- If you’d rather not use your real name, that’s okay — just tell us.
- Photos are a separate permission. This form is about your written story only.
This is your choice
- You don’t have to do this. Saying no is completely okay, and it won’t affect anything else about working with us.
- You’re not being paid for your story, and you’re not paying us. This is about sharing your experience to help other people.
You can change your mind
- Even after your story is published, you can ask us to take it down. Email contact@created4.work and we’ll remove it from our website.
- Once something is public, other people may have already saved or shared it, and those copies can be hard to reach — but we’ll remove everything we control.
Your contact information
- We ask for your email (and phone, if you’d like) so we can reach you about your story.
- We won’t sell your information, and we won’t use it for anything except contacting you about your story.
If someone is signing for you
- If a parent, guardian, or conservator is signing on your behalf, they are giving this permission for you.
- We’d still like you to agree too, as much as you’re able — this is your story.
